Mountain States' new Health Management Program uses trained health coaches to work with participants on medications and other ways to be healthier and reduce costs.

New Mountain States Health Management Program offering free diabetes meds, supplies

Mountain States team members and dependents with diabetes could receive medication and supplies at NO COST, effective July 1, 2017.

 

This is part of a new Health Management Program being offered by Mountain States Human Resources. If you enroll in the program, you can get your diabetes medications and supplies at no cost through any Mountain States Pharmacy. The program is open to team members and their dependents enrolled in the Mountain States medical and pharmacy plan.

 

You can enroll now – or at any time during the year – but you must be enrolled in the program for at least three business days prior to going to the pharmacy to pick up your prescriptions and supplies.

 

As an added convenience, our pharmacies deliver to Mountain States locations. To arrange for delivery or to transfer your prescriptions, call 423-926-6154.

 

IMPORTANT NOTE: With this program, the no-cost diabetes medications and supplies you receive will not count toward your deductible or require you to pay coinsurance.

 

 

 

MORE ABOUT THE HEALTH MANAGEMENT PROGRAM

 

We’re starting out by offering the Health Management Program to people with a diagnosis of type I or type II diabetes, but in the future we plan to expand the program to include other diagnoses beyond diabetes.

 

This program uses trained health coaches to work with participants to set goals and to communicate with other health care providers (including your primary care physician). The goal of this collaborative effort is to help participants be healthier, reduce the participant’s out-of-pocket costs and manage the health plan costs.

 

How do you enroll? Complete and submit the online enrollment form, found on the HR and Benefits SharePoint sites. This will indicate your interest in the program. A health coach will contact you to set up your initial meeting.
